The pancreas is the organ responsible for the secretion of the hormones insulin and glucagon. Insulin is a hormone that regulates glucose levels, allowing the body's cells to absorb and use glucose. Without it, glucose cannot enter the cell and therefore cannot be used as fuel for the body's functions. If the pancreas is exposed to persistently high elevations of blood glucose levels, the insulin-producing cells in the pancreas could be damaged, causing a lack of insulin in the body. Insulin resistance occurs when the pancreas tries to produce more and more insulin in response to persistently elevated blood glucose levels. Eventually, the rest of the body becomes resistant to the insulin that the pancreas is producing, thereby requiring more insulin to achieve the same blood glucose-lowering effect, and forcing the pancreas to produce even more insulin to compete with the resistance. This negative spiral contributes to pancreatic burnout, and the disease progression of diabetes. To monitor the body's response to blood glucose-lowering therapy, glucose levels can be measured.

=== Lasso peptides === Lasso peptides are short peptides containing an N-terminal macrolactam macrocycle "ring" through which a linear C-terminal "tail" is threaded. Because of this threaded-loop topology, these peptides resemble lassos, giving rise to their name. They are a member of a larger class of amino-acid-based lasso structures. Additionally, lasso peptides are formally rotaxanes. The biosynthesis of lasso peptides is different from other RiPPs because the N-terminal leader peptide is cleaved first by a dedicated peptidase. After the cleavage, a lasso cyclase enzyme forms an isopeptide bond between the N-terminal amine of the first amino acid of the peptide and the carboxylate side chain of an aspartate or glutamate residue. This creates a constrained lasso peptide with three sections: the ring, the loop, and the tail. The ring is typically 7 to 9 amino acids long, while the C-terminal tail ranges from 7 to 15 amino acids in length. The first amino acid of lasso peptides is almost invariably glycine or cysteine, with mutations at this site not being tolerated by known enzymes. Thus, bioinformatics-based approaches to lasso peptide discovery have thus used this as a constraint. == Advantages == There are many advantages to the use of microneedles, the most prominent being the improved comfort of patients. Microneedles are also less expensive than needles as they require less material and the material used is cheaper than the materials in hypodermic needles. Microneedles present a new opportunity for home and community-based healthcare. One of the biggest drawbacks of traditional needles is the hazardous waste that they produce, making disposal a serious concern for doctors and hospitals. === Degradation of collagen === UV exposure would also lead to the activation of receptors for epidermal growth factor, IL-1, and TNF-α in keratinocytes and fibroblasts, which then activate signaling kinases throughout the skin via an unknown mechanism. The nuclear transcription factor activator protein, AP-1, which controls the transcription of matrix metalloproteinases (MMP), is expressed and activated. MMP-1 is a major metalloproteinase for collagen degradation. This entire process is aided by the presence of reactive oxygen species that inhibit protein-tyrosine phosphatases via oxidation, thereby resulting in the up-regulation of the above-mentioned receptors. Another transcription factor, NF-κB, which is also activated by UV light, increases the expression of MMP-9. The up-regulation of MMP can occur even after minimal exposure to UV; hence, exposure to UV radiation that is inadequate to cause sunburn can facilitate the degradation of skin collagen and, presumably, lead to eventual photoaging. Thus, collagen production is reduced in photoaged skin due to the process of constant degradation of collagen mediated by MMPs. In addition, the presence of damaged collagen would also downregulate the synthesis of new collagen. The impaired spreading and attachment of fibroblasts onto degraded collagen could be one of the contributing factors to the inhibition of collagen synthesis.
==== Non-collagen ==== Hydroxyproline is found in few proteins other than collagen. For this reason, hydroxyproline content has been used as an indicator to determine collagen and/or gelatin amount. However, the mammalian proteins elastin and argonaute 2 have collagen-like domains in which hydroxyproline is formed. Some snail poisons, conotoxins, contain hydroxyproline, but lack collagen-like sequences. Hydroxylation of proline has been shown to be involved in targeting Hypoxia-inducible factor (HIF) alpha subunit (HIF-1 alpha) for degradation by proteolysis. Under normoxia (normal oxygen conditions) EGLN1[1] protein hydroxylates the proline at the 564 position of HIF-1 alpha, which allows ubiquitylation by the von Hippel-Lindau tumor suppressor (pVHL) and subsequent targeting for proteasome degradation. This isotope does not cause aging of the phosphor, as alpha emitters do, and therefore the light emission is stable for a few years. Originally, radium-226 was used for the purpose, but it was later replaced by promethium-147 and tritium (hydrogen-3). Promethium may be favored over tritium for nuclear safety. In atomic batteries, the beta particles emitted by promethium-147 are converted into electric current by sandwiching a small promethium source between two semiconductor plates. These batteries have a useful lifetime of about five years. The first promethium-based battery was assembled in 1964 and generated "a few milliwatts of power from a volume of about 2 cubic inches, including shielding". Promethium is also used to measure the thickness of materials by measuring the amount of radiation from a promethium source that passes through the sample. === Unnatural amino acid incorporation === Cell-free systems have also been used to incorporate unnatural amino acids. Shimizu et al. were able to change a stop codon to a sense codon by omitting the RF1 release factor, indicating ability to insert desired amino acids in unnatural situations. The bactericidal effects of β-lactam antibiotics are achieved through inhibition of the bacterial cell wall synthesis. The cell wall of both gram-positive and gram-negative bacteria is a tight covalently bound and cross-linked peptidoglycan network and essential for bacterial growth, cell division and cellular structure. Therefore, bacteria need enzymes that can cleave the cell wall during bacterial growth and cell division. The cell wall of bacteria is built up in two steps from the outside of the cell. In the first step, molecules of disaccharide units linked with peptides on their ends are transported from the cytoplasm of the bacteria and joined on the outside of the wall by a transglycolase. In the second step, a transpeptidase links together long polysaccharide chains which are linked together through peptide bonds. The amino acid sequence of D-alanyl-D-alanine is recognized by the transpeptidase at the end of the peptide chain. The enzyme cleaves off the alanine on the terminal end and joins the remainder to a peptide chain from an adjacent polysaccharide. This transpeptidation reaction is inhibited by β-lactam antibiotics like cephalosporins. Because of this inhibition the antibiotics are most effective when the bacteria are in the logarithmic phase of growth, where then they are synthesizing the cell wall. If the bacteria are in the stationary phase of growth, then there is no wall synthesizing in progress, and the antibiotics have much lower effect.
=== Pseudohyperkalemia === Pseudohyperkalemia occurs when the measured potassium level is falsely elevated. Mechanical trauma during blood drawing can cause potassium leakage out of the red blood cells due to haemolysis of the blood sample. Fist clenching during the blood draw can cause a rise in potassium levels in the venous blood as it is sampled; this difference may be as much as 1 mmol/L. Differences of this order of magnitude cause problems (false positive results for clinically-important hyperkalemia) for patients with low glomerular filtration rate (GFR; a measure of kidney function), type IV renal tubular acidosis (RTA), or on evidence-based medication for cardio-renal risk (RASi, MRAs). The practice, widespread in laboratories in North America, should be discontinued. Prolonged storage of blood samples or agitation in transit is also associated with red cell lysis that can increase serum potassium levels. Hyperkalemia may become apparent when a person's platelet concentration is more than 500,000/microL in a clotted blood sample (serum blood sample). Potassium leaks out of platelets after clotting has occurred. A high white cell count (greater than 120,000/microL) in people with chronic lymphocytic leukemia increases the fragility of red blood cells, thus causing pseudohyperkalemia during blood processing. This problem can be avoided by processing serum samples, because clot formation protects the cells from haemolysis during processing.
=== Initial discoveries === Theories on the coagulation of blood have existed since antiquity. Physiologist Johannes Müller (1801–1858) described fibrin, the substance of a thrombus. Its soluble precursor, fibrinogen, was thus named by Rudolf Virchow (1821–1902), and isolated chemically by Prosper Sylvain Denis (1799–1863). Alexander Schmidt suggested that the conversion from fibrinogen to fibrin is the result of an enzymatic process, and labeled the hypothetical enzyme "thrombin" and its precursor "prothrombin". Arthus discovered in 1890 that calcium was essential in coagulation. Platelets were identified in 1865, and their function was elucidated by Giulio Bizzozero in 1882. The theory that thrombin is generated by the presence of tissue factor was consolidated by Paul Morawitz in 1905.
